Please use the following form to provide the school with your most current, up-to-date Emergency Contact Information. This form is required of all students. Return this form to the Principal's Office.
Our district enjoys featuring your child's accomplishments using positive PR including local newspapers, school website or organizational Facebook pages. If you don't wish your child to be included in this, please revoke consent using the following form.
Parents of a child requiring medication to be given at school for any reason will need to fill out this form. Return this form to the Principal's Office if your child need's to have medication distributed at school.
Parents of a child transferring from any district in-state or out of state will need to complete this form for us to obtain your child's school records from their previous school. This form will be needed if your child was homeschooled as well. Return this form to the Principal's Office.
Students living within 1 mile of the district are considered within walking distance if in grades 3-6. Students living within 1.5 miles of the district are considered within walking distance if in grades 7-12. K-2 Students are to ride the bus unless alternative arrangements are made such as parent "pick-up". Return this form to the Principal's Office if your child will need school transportation.
Please fill out the proof of residency form when enrolling your child. You will need to provide the district with proof of residency such as drivers license, utility bills, renters agreement/lease, or other verifiable documentation. Proof of residency documentation must be verified in the Principal's Office.
Please consider completing the release of information consent form for your child. This will permit confidential exchange of information regarding your child's medical information between your child's health care, community support providers and the school. This consent form also allows the school to complete evaluations on your child to provide the best possible systems of support for their educational success.
